1. Will Driver’s License numbers remain the same after an individual re-news and receives a Utah Driving Privilege Card? Yes, the number assigned to the person regardless of card type or whether it is a re-newel or duplicate will remain the same.

2. Does an individual need to re-test in each area yearly, required for driving (vision, road, written)?
Yes a person will need to re-test the vision test every year.Written tests will depend on driver history.If more than four tickets or if the privilege has been suspended or revoked in the previous five years, a written test will be required.Road tests are based on the driver history.If the privilege has been revoked in the previous five years, a road test may be required.

3. Where does the money go from the yearly re-newel of the Utah Driving Privilege Card? The monies obtained from the Utah Driving Privilege Card will go towards Administrative costs. This was decided by Legislation.

4. Does an individual need to bring the actual ITIN or SSN card when they come in to re-new cards?
Yes, the actual ITIN or SSN card is required in order to issue any forms of identification by the Drivers License Division.

5. If an individual loses or has their card stolen, do they need to reapply for a new card or can they obtain a duplicate card? Individuals will need to reapply if their card is lost or stolen.The current expiration date will determine if the card is a duplicate or renewal.

6. What documents and/or information must be provided to verify an individual’s residence?
In order to verify a residence address as the individual’s place of domicile or an address where the individual resides as a resident, an application for a Utah driver license, Utah driving privilege card, or Utah identification card shall be accompanied by a minimum of two different types of original (current and valid) documents from the following list identifying the individual’s name and residence address:

(a)Property tax notice, statement or receipt, within 1 year
(b) Utility bill, billing date within 60 days, no cell phone bill
(c) Utah Vehicle registration or title, current not expired
(d) Bank statement, within 60 days
(e) Recent mortgage papers
(f) Current residential rental contract
(g) Major credit card bill, within 60 days
(h) Court order of probation, order of parole or order of mandatory release, must display residential address on the order
(i)Transcript(s) from an accredited college, university, or high school

The division may also consider other documentation furnished by the individual if it can be determined that the documentation unequivocally demonstrates proof of residency or domicile.
